The Patient Health Questionnaire 9 (PHQ-9) is a screening and monitoring test for depression. It is not a replacement for a diagnosis from a doctor. The answers to each question are given a value from 0 to 3 depending on severity. Scores range from 0 to 27, with higher scores indicating more anxiety.
